Insurance applicants/consumers

We may have a prescription report about you only if you authorized the release of your medical records to an insurance company and that company submitted a request to us. Otherwise, we would have no information about you. Please contact us to get a copy of your report.

Please note that each individual 18 years of age and older will need to contact us separately to receive his/her report due to privacy regulations. Your privacy is important to us.

Please be prepared to provide the following information:
• Full Name (please spell your name if you’re leaving a message)
• Mailing Address (and e-mail address if you’d like to receive your report via e-mail)
• Phone number (we may call you if we have questions)
• Date of birth
• Last four digits of SSN
• Insurance company to which you applied for insurance
