Our process is protected by sophisticated security measures. Your information is retrieved in real time and securely delivered to the insurance company you authorize. This data is only accessible to your insurance provider for the purpose of processing your application or, in some cases, evaluating a claim.

To request a copy of your report, please provide the following information (using one of the methods on the right).

  • Your first and last name
  • Your daytime contact number
  • Your mailing address
  • Your date of birth
  • The last four digits of your Social Security number
  • An email address if you want the report emailed to you
  • The insurance company that processed your application or claim

 

Please know:

  • Milliman IntelliScript will only have information about you if you have authorized the release of your records to an insurance company and that company asked us to gather your records.
  • Milliman IntelliScript will not have a copy of your report if you have not applied for insurance in the past seven years or authorized release of information in connection with a claim.
  • Milliman IntelliScript does not make the decision to grant or deny insurance. Milliman IntelliScript is not an insurance company, and we don’t have any information about the insurance company’s decision.
  • If you are looking for information about your insurance application or the decision made about your insurability or a claim, please contact your insurance company or agent.
  • Your privacy is important to us. Individuals 18 years of age and older must request their own reports.
